Library Card Applications
To apply for a library card, please choose from the forms below and bring the completed form into the library to get your card. Cards for persons aged 17 and under must be filled out by an adult parent/caregiver. The adult parent/caregiver must also be present when the card is issued.

To receive a temporary library card, please email the library director Erin at ebusbea@lowndes.lib.ms.us for more assistance in setting up a temporary library card if needed.
